Register by May 10^th, 2003 to receive discount – All registrations received after May 10^th will pay $190.00 per participant.  We will accept registrations at the site on Thursday only

          Participant Fee before May 10^th, 2003       $165.00 includes banquet

                    After May 10^th - $190.00 per participant

 

          Club Discount Fees:  please fill in a separate form for every person that you are registering from your club.  Send all forms together with a club check to receive discount. Registrations must be received by May 10^th, 2003. 

          1^st Participant - $165.00  includes banquet

          2^nd Participant - $150.00  Includes banquet

          3ed or more participant - $130.00 or $100 with NO Banquet

Indicate on each form if participant will attend banquet and include proper fee

No Refunds after May 10^th, 2003 – refunds will be given before May 10^th minus $25.00 fee
